  • - Educational Contract
  • - GDPR Agreement
  • - Enrollment Form
  • - Copy of the child’s birth certificate
  • - Copies of parents’ ID cards
  • - Two passport-sized photos of the child
  • - Medical file
  • - Copy of the child’s vaccination record
  • - Epidemiological clearance stating "FIT TO JOIN THE COMMUNITY"
  • - Medical certificate for enrollment in the community with the note "FIT FOR PHYSICAL EDUCATION AND SPORTS"
  • - Copy of the Medical Consultation Sheet, including all known conditions, allergies, etc.
  • - Children’s lab reports: stool test (coproparasitological exam) and throat swab
